alakov on "[Plugin: Contact Form 7] required fields problem"

Hi!
I have a problem with contact form 7 on http://woorirussia.com/contact-us/.
Name, email, topic fields cannot be filled out there. I noticed this issue yesterday, but latest email was received a week ago or so.
Where is the problem and how to fix it?
