ajamot on "Cannot Make two columns with Contact Form 7"

Hi everyone,
I cannot make my form looks like this one with the googlemap in a left column and the form in the right one : http://themes.devatic.com/daisho/contact/

I've tried to customize it as follow by making 2 columns but doesn't work. Here the code of my contact page :
